The NPSA have issued advice on risks with intravenous heparin flush solutions (Rapid Response Report April 2008) in response to patient safety incident reports concerning mis-selection of sodium heparin products and reports where other medicines were mis-selected for heparin flush solution products.(www.npsa.nhs.uk/patientsafety)
2. Sodium chloride injection should be used instead of heparin to flush peripheral lines.
3. Lepirudin is used in heparin induced thrombocytopenia. Wards would normally order it through pharmacy on a named patient basis.
4. RCHT policy is as follows:
Dalteparin should be used for prophylaxis and treatment of thromboembolic disorders, treatment of unstable coronary syndrome, and treatment of pulmonary embolism.
